From my 3 years of teaching I have collected hundreds of worksheets and game templates.  I have also managed to past the art of PowerPoint. Unfortunately, I haven't figured out how to upload my PPT's onto the web; however, I have a few gems in the form of ESL websites which can help you in your classroom.

http://www.bogglesworldesl.com is an excellent site to find games and worksheets for younger learners.  it also has great conversation starters for adult students.

http://www.esl-kids.com/eslgames/eslgames.html is a fantastic site for elementary level students; however, some of the worksheets can be used to teach low level middle school students.

http://www.genkienglish.net/ is game treasure chest.

http://www.dltk-kids.com/ is one of my favourite sites to find easy craft ideas and templates.

http://www.esl-galaxy.com/themesheets.html as both worksheets and PPT templates.

http://busyteacher.org is by far the BEST resource I've ever come across. Aside from a plethora of worksheets, games and activities they also offer wonderful tips on teaching and classroom management.

http://www.eslprintables.com is super helpful for worksheets and a few PowerPoints. However, they require you to contribute in order for you to access files. Once you make your first contribution you'll be able to gain access to all their files.
